Hey Toby,
There are a few pay-at-the-door lounges (Bidvest or Shongololo), they are ok but not great. I would highly recomend you to leave the airport, catch the Guatrain out the airport (Very safe & Easy) to either Sandton or one of the Stations, and go to a restraunt or shopping.

You can book a day room at the Intercontinental too - just over the road from the JNB terminals, 2 min walk. Gautrain to Sandton also as good as saul says, not like other trains in the Joburg area.

The Intercontinental can be quite expensive. There are also several other hotels that are a short 5 minute shuttle ride away including the Protea Hotel, and Southern Sun Hotel.
The Gautrain takes around 20 minutes each direction, plus waiting times, etc., could take up to 40 minutes each direction. So if you have a 6 hour wait, plus need to be back to JNB 2 hours prior to your flight, would only give you less then 3 hours in Sandton.
