FWIW: for Singapore the Singtel hi!Tourist SIM card would be worth a look. In Malaysia (well, just KL) I've used MAXIS Hotlink, available at the airport.
Singapore's M1 now allows you to roam for free in Malaysia (also HK, Indonesia, Macau and Taiwan) whilst consuming your local (Singapore) data allowance - this applies to their M Card Prepaid SIM. To activate roaming, just dial #100*8*1# from your phone.
When you're in Malaysia, the M1 SIM will roam onto Malaysia's Celcom network which has EDGE, 3G as well as 4G LTE. I've been using this a few times now - sometimes it will take a few minutes to roam onto Celcom (you might need to toggle flight mode a few times) but it works a treat.
Alternatively (and it may work out to be cheaper), you can do what watson374 suggested and just buy 2 local SIM cards.
Wow, thanks for all the info. Newbie first time with iPhone traveling. Only a few days in Singapore but a week in Malaysia, so might hold off for Sabah vist but will checkout M1. Thanks
Just a follow-up if anyone is still following. The M1 sim worked out great for me. Went an M1 mall store after arrival, staff were helpful, loaded additional data before leaving for Malaysia. Celcom connection film was seamless. Worked great, thanks for the help

Sim card for Singapore and Malaysia? I will be in Singapore and Malaysia in 2 weeks. Can I buy a sim card and data for both countries? Many thanks!

No, Singapore and Malaysia have different telcos and they put you on roaming when you use a Malaysian SIM in Singapore (and vice versa).
You will need to present a passport to get even a basic prepaid SIM in both countries.
If you're not going to be there very long, it might not even be worthwhile.
